A - No meetings Friday, every meeting had to have an agenda, DACI decision-making framework, if you're in the office and somebody was dialing in, then you dialed in through your laptop, just little prompts around the space to remind people - Tracy Hawkins

Giving permission that if you turned up to a meeting and was a whole bunch of your team there, to actually reassess and say, 'Hey, do we all need to be here? Or can some of us go work on something else?' - Tracy Hawkins

And really good hygiene around your calendar, looking at it on a regular basis and saying do all these meetings still make sense? Should I be getting rid of some of these? Can they be replaced by asynchronous work? - Tracy Hawkins

Give people permission to take control of their calendars. 
A clear understanding of everyone's role in getting to a decision
Frequent reassessment of 'standing' meetings
Giving permission, that if the team's adequately represented in the meeting, cut the extra people loose to work on something else.

Simple actionable steps to improve meeting efficiency.
